Position Description
The Geriatric Mental Health Outpatient Services is currently seeking a full-time, permanent Social Worker 2 to work in the Complex Care and Recovery Program. Reporting to the Senior manager of Geriatric Mental Health Outpatient Services, you will be expected to work collaboratively within an interprofessional outpatient team setting. Your target population of clients will be 65 years of age and older, with mental health and addiction issues, often with co-occurring medical issues. You will provide in-depth psychosocial assessments, along with other clinical assessments, with patients and families and will offer appropriate interventions and recommendations through the development of integrated care plans. You will provide or co-facilitate a combination of group and individual counselling sessions to clients, as appropriate, and collaborate with the interprofessional clinical team in the formulation of individualized client care plans. You will also provide education and financial or social supports/assistance to clients, family and caregivers from diverse backgrounds. As such, you will demonstrate your expertise in liaising with community agencies and established community partners to address client housing, income and other care needs. Your focus will be to deliver an exceptional patient experience and create seamless care transitions by running psychotherapy groups including but not limited to CBT and CBSST. The successful candidate will leverage their strong coordination, communication and collaboration skills to facilitate successful follow-up in the community. You will be expected to participate in program planning, program evaluation, quality improvement, and research activities. Candidates must be capable of providing supervision to Masters of Social Work students, collaborating with faculties of social work on special projects related to the development of the social work field, contributing to the development of best practices unique to the social work field and also teaching to other professionals. This position will require some evening and weekend hours. You will support a healthy workplace that embraces diversity, encourages teamwork and complies with all applicable regulatory and legislative requirements. This position is located at 1001 Queen Street West (80 Workman Way).

Qualifications
The successful candidate will possess a Master’s of Social Work degree combined with a preferred 3 years of post-graduate work experience and be registered and a member in good standing with the Ontario College of Social Workers and Social Service Workers. You must be eligible for an academic cross-appointment. Proven experience in Social Work with geriatric clients, knowledge of the aging process and knowledge of geriatric care issues are required. Previous experience in working in a psychiatric/mental health setting, along with knowledge of Psychosocial Rehabilitation and Recovery principles, would be considered an asset. In addition, you possess a fundamental knowledge and application of current best practices for the Geriatric Mental Health Service clients. The ability to provide culturally competent services to clients of diverse ethno-racial and cultural backgrounds is essential and experience working in a multi-system, inter-professional environment is required. Excellent leadership, organizational, interpersonal and written communication skills and the ability to work effectively in a fast-paced clinical environment are also essential. Computer skills, including word processing, electronic communication and workload measurement are essential. Bilingualism (French/English) or proficiency in a second language would be an asset.

Please Note: This full-time, permanent position is part of the OPSEU bargaining unit.

Salary Range: $36.04 – $48.57 per hour
