Outreach Worker

London InterCommunity Health Centre

This is a Full-time position in Toronto, ON posted July 31, 2018.

The London InterCommunity Health Centre is looking for an Outreach Worker to support our clients. This role works as a part of an interdisciplinary team. Our goal is to address the needs of Londoners who experience barriers to accessing care and services and have complex addiction and/or social issues. This role is responsible for engaging clients through street level outreach and in-reach at partnering agencies, as well as educating individuals and communities, primarily through:

  • Prevention and harm reduction education
  • Offering HIV testing: reaching as many people as possible (full training available, no experience required)
  • Supporting client engagement in their Hep C and/or HIV treatment and care plan.

This role is of a permanent nature and would require full time hours (37.5 hours/wk).
We want to hear from you if you:

  •  Are committed to harm reduction (i.e. meet people where they are at, absence of judgment) 
  •  Have working knowledge of addictions and have used harm reduction principles when supporting people
  •  Have excellent interpersonal and problem solving skills with strong written and oral communication skills
  •  Have experience with both individual and systemic advocacy
  •  Have crisis intervention/de-escalation skills
  •  Demonstrate strong organizational, time management skills with strong computer and data management   skills
  •  Have knowledge of community resources and London social service agencies


  •  Direct experience with communities at risk
  •  Post-secondary education in social services and/or experience providing education/support/counseling to   adults
  •  Minimum three (3) years’ experience working within the harm reduction model of service delivery
  •  Knowledge about HIV/HCV transmission, social determinants of health related issues
  •  Knowledge of different substances and how they may impact on a person’s behavior
  •  Valid driver’s license for work-related duties preferred
  •  Training and rigorous application of safe sharps handling practices
  •  Basic First AID, CPR, Crisis Prevention Intervention and will obtain a Vulnerable Positions Screening (Police   Check)
