Intensive Case Management – Activity Programming

Contract Position – Six Months in Length (3pm to 11pm, Monday to Friday) 

Mission Statement: Homes First Society develops and provides affordable, stable housing and support services to break the cycle of homelessness for people with the fewest housing options.

This position supports clients in their housing by assessing and supporting their needs by organizing and coordinating activity programming on site for residents while adhering to the mission statement and policies and procedures of Homes First Society.

Job Description:

  • Create, coordinate, organize, promote and implement a series of activities, programs and developmental services and ensure they are successfully carried out for the tenants/residents of Homes First, with the overall goal of building life skills and community for the residents on site.
  • Review intake information to determine program needs.
  • Coordinate the program supplies and work within a budget.
  • Ensures safety policies and procedures are applied consistently in programs, in consultation with the site supervisor.
  • Demonstrated ability to work with people who have mental health and addiction issues.
  • Work with the Volunteer Coordinator to utilize volunteers.
  • Organizing programming to provide support to residents, including work readiness, resume building, basic life management skills, anger management, self-esteem, conflict resolution, problem solving and managing change.
  • Remain knowledgeable and up-to-date regarding trends, issues, and information relevant to the tenant population in order to provide meaningful programming.
  • Provide support to clients who are facing issues of poverty, mental health, addictions, abuse, social isolation and more.
  • Use case management as a tool to support residents in meeting their goals.
  • Record best practices and collect data for evaluation purposes.
  • Participate in staff training and orientation.
  • Maintain client records professionally and confidentially in accordance with Homes First standards.

Qualifications:

  • Minimum three years experience in related post secondary education or equivalent combination of education and experience.
  • Experience providing group/individual support for those who are homeless, living in poverty, dealing with severe mental health issues and addictions, socially isolated and who come from diverse racial, ethnic and cultural backgrounds.
  • Experience with conflict resolution and negotiation skills.
  • Experience in developing and carrying out programs and activities.
  • Highly developed written and oral communication skills.
  • Proven ability to plan and prioritize work, to adapt to changing conditions and to develop creative solutions to problems.
  • Able to trouble-shoot and resolve issues as they arise
  • Thorough knowledge of community based resources.
  • Demonstrated case management experience
  • Highly developed group facilitation skills.
  • Highly developed understanding of and ability to communicate appropriate professional boundaries.
  • Highly developed ability to prioritize work through short and long term goals and remain flexible.
  • Literacy in Microsoft Office.
  • Able to work independently and in inter-dependent teams, with minimum supervision in a busy and flexible environment
  • Certificate in First Aid, CPR an asset
  • Excellent organization and time management skills
  • Ability to work evenings and weekends

Working in an environment where there may be smoke is required.
